Quarterly Notification of Military Member Using Tuition Assistance

Each quarter, after you have made the appropriate request through your service to use tuition assistance, please use this form to notify us that you will be using these benefits.

Tuition Assistance is available to most active-duty military members. In general, you must have served a minimum of 181 days on active duty before you are eligible for Tuition Assistance Benefits and will need your supervisor's permission.
Some service branches are first-come, first-served. If you have an upcoming due date, please let us know.
